In order that we may be assured that the dog or cat you are applying to adopt will be best suited to your home and lifestyle, and that it will be placed in an environment that is compatible with its needs, would you please fill out this form and return it to us as soon as possible.

Please think carefully about this adoption. If you are not prepared to make a lifetime responsible commitment to this pet, then the best solution is for you to wait till you aer ready to make this commitment. We assume no liabilities concerning the information given us by the original owner. We pass any information on to you as the owner gave it to us. If the pet was a stray and we purchased the pet from one of the many animal shelters we deal with, we will tell you what we know after working with and raising this pet. We socialize all pets with dogs, cats, kids and strangers as much as possible. We crate train the dogs and litter train the cats in order for them to continue to be indoor only pets in your home. No outside pets will be allowed. We have taken care of all the medical expenses and taken care of all expenses for inoculations, spaying or neutering, deworming, testing as needed, health exam and certificates, and microchip. We never get any pet spayed or neutered if it is under 3 months of age. Most of the time we do not know the pet’s real name, so we give the pet a name for identification purposes in the computer. You are welcome to keep or change the pet’s given name. You should allow at least four weeks for the pet to bond to you and your family, and not allow it to go outside unless on leash or in a pet carrier especially since they don’t know where they are and have never been let off leash and for cats, never been outside. It is a good rule to designate one room for them at first that they can become housetrained in, feel safe and stress free, and there will also be less chance of a front door being left open accidentally for them to get out and get lost or hit by a car. You must keep dogs on leash at all times unless in the house or in the fenced back yard that is safely locked at the gate. Even the best trained dogs have been tempted by something and ran out in front of a car. Inside trained dogs and cats stay healthier and have less chance of contracting disease, train much faster, learn less bad habits like barking, digging, chewing, or running away, and bond better with their family. You must contact Save A Dog & Kids right away no matter what time of day if you do lose your pet so we may be in contact with you and the animal control/shelter personnel in your area who will contact us with the microchip number, and also so we can advise you of all you can do to find the pet right away. We have 10 years plus experience with that and have recovered every microchipped pet. If you fail to call us and we are called by any animal control to come pick up the pet and pay the impound fee, and any costs to have someone pick them up including a mileage charge of not less than $25, and we will do a determination of whether the pet is being protected from running into the street properly or take the pet back, you will also be charged these fees. We will do everything in our power not to allow any of our rescued pets stay in a shelter more than one day. If you refuse to pick up the pet the day we or the animal control notify you, you will also be charge these fees, and we will take the dog back. You will receive shots, spay/neuter, microchip certificates. They will tell you when the next shots are due. If you return the pet, then you must return the medical papers to us. If you have lost the papers you will be charged $35 for the replacement cost. If you have lost your rabies tag then you must contact the Clinic and get a replacement when you return the pet. Did you know that you should never take off your pet’s collar and tags, not even for a bath. One of our recent adoptive families learned the hard way, and now she will never remove his tags since she was very lucky to have him returned after much worry. If he had been wearing his tags with her phone number, the people that found and kept him for a while would have called her right away.

IT MUST ALWAYS COME BACK TO SAVE A DOG & KIDS EVEN IF YOU NEED TO PAY FOR SHIPPING THE DOG OR CAT BACK ON DELTA AIRLINES FOR $167.50 IN A PET CARRIER. The microchip in your pet stays in our name with our 24 hr number. Shelters are more willing to work with rescues to keep the pets alive until the owner can be found. You need to keep us updated with changes in address and phone numbers throughout the life of your pet . Save A Dog & Kids 24 hour 7 day a week number has been payed for with Voicestream now T-Mobile for the past 5 years just to be there for the pets. We also require that you purchase a tag made at Petsmart for $6 with your last name, street address and 24 hour phone number before you get your pet, a buckle not snap collar, leash for dogs and cat carrier for cats, and any other supplies you will need. Never feed your dog or cat table scraps, since they have been so inbred over the years, their organs can no longer digest people food and get many allergies like itching, hot spots, over weight and later cancer if fed foods high in corn, preservatives, artificial color and taste. HEY JUST LIKE PEOPLE! We do not recommend any foods that contain CORN or do testing on animals like Science Diet.

 

MISSING OR MISPLACED ANIMALS: AGAIN NOTIFY US IMMEDIATELY ANYTIME! We have pictures of you and your pet, we also keep good working relationships with shelters and are experienced in searching for lost pets.

 

MEDICAL RELEASE OF LIABILITY: Save A Dog & Kids will not be responsible for any medical bill for your pet after adoption unless arranged for at adoption. If your pet becomes ill and the bill is too much for you to bare, please contact us. There are funds that help with these for low income families in most communities. You can also help build up this fund by donating your tax deductible donation to Save A Dog & Kids for all needy pets.

 

WE RESERVE THE RIGHT TO DO SURPRISE HOME CHECKS DURING THE LIFE OF THE PET YOU ADOPT FROM US TO ENSURE THE PET IS PROPERLY BEING CARED FOR ACCORDING TO OUR GUIDELINES. WE RESERVE THE RIGHT TO TAKE BACK ANY PET THAT IS FOUND CHAINED OR TIED UP, ABUSED OR NEGLECTED IN ANY WAY, OR NOT BEING PROPERLY CARED FOR.  I/we further understand and agree that: 

  1. To return the dog to the Rescue Program if unable to keep the dog. To never abandon the dog or surrender, sell, or give the dog to a shelter, pet dealer, or a laboratory.
  2. To notify the Rescue Program if the dog is lost or stolen and to make all reasonable efforts to locate the dog.
  3. To never allow this dog to be trained for or used for protection or as an attack dog.
  4. To treat this dog as a family pet with affection and kindness, never subjecting this dog to abuse or cruelty.
  5. To provide regular veterinary care, including annual shots, heartworm preventative, and parasite checks.
  6. To always keep this dog within a fenced yard (not on a chain), on a leash, or safely within the home.
  7. To not transport this dog unconfined in an open vehicle.
  8. To regularly check the tightness of any collar. To affix any licenses and tags to the collar and to never leave a choke chain on the dog when not attended.
  9. To notify the Rescue Program of the dog's annual progress.
  10. To allow the Rescue Program to periodically check on this dog.
  11. To contact the Rescue Program, a veterinarian, or an obedience trainer when problems arise so that the problem may be eliminated or alleviated.
